Fieldwork or Student Teaching Request
Candidates complete one form for the district/school in which they are requesting to complete their Fieldwork and Student Teaching Experiences. District web sites and data dashboards contain much of the information requested. Prior to student teaching, candidates must complete a total of 200 hours of fieldwork including a minimum of 50 hours of observation/participation in general education and 150 hours in special education environments. Candidates document their experiences towards meeting these requirements through Time2Track

District Demographics: Describe the race, ethnicity, cultural backgrounds of the students in the district. *
District Demographics: Identify the languages and percentage of student speaking the specific languages. *
District Demographics: Describe socio-economic characteristics of the students in the district.  Identify content related to students below the poverty level, number of Title I schools or other content indicating that the district serves students from low income households. *
District Demographics: Students receiving special education services. Include process for service delivery. *
Name the specific schools within the district in which you will  participated during the Fieldwork ll and/or Final courses. *
Identify the district curriculums for reading/language arts, mathematics, science, social studies.  Identify any alternative curriculums. *
Present your rationale for the request.   *
A copy of your responses will be emailed to the address you provided.
