1. This is an initial step and there will be other forms to complete on your first day of school to finalize your enrollment process.

2.Please complete and SUBMIT this form by clicking submit at the bottom. After Submitting the form, please CALL the admissions office at 469-312-8885 to pay $100 deposit, towards your tuition over the phone. This amount is to reserve your spot in the class. It is applied towards your tuition. It is not an extra cost.Ā  The deposit is refundable up to 0330pm, 3 business days before your class start date. After you have made the payment, please call the admissions office at 469-312-8885 to get further instructions.

3. The application form alone without the reservation payment does not reserve your spot in the class. Ā Please reserve to get the date that you desire. Our classes fill up quickly from walk in students and other online applicants.

4. You will need to bring your ID, SS card and High School Diploma or unofficial high school transcript to the school on the your first day of School. Ā 

Ā 5. If you do not have proof of high school you may bring a college diploma, an unofficial college transcript or GED. Foreign Documents are accepted. šŸ¦ŗšŸ—šŸš§ This requirement can be waived if you will be getting your GED or high school diploma within the next 12 months. Please call the office at 469-312-8885.

6.You may wait to bring your documents on the first day of school if you have already paid your $100 reservation fee and are not able to come to the school before the first day of your class.

7.The school accepts walk-ins on Monday to Friday from 9:30am to 3:00pm and every Saturday from 9:30am to 12:30pm

9.Ā Our coursesĀ are unique blend of lecture and lab work to get you experienced with a variety of venipuncture and Ā Iā€™ve therapy techniques.

10.Ā PRACTICAL EXAMĀ One-on-one practical exam.Ā On our last days of class, we cover questions, specific scenarios, and finish with a one-on-one practical exam to go over procedures learned in class. Legacy Career Institute is registered to proctor your state exam for you on site.Ā

The 4 days in class are followed by 3 days at home, reviewing study videos and doing practice tests.Ā Most students complete the 4 day program, study modules and state testing within 7 days. More time is given if student needs it.Ā TUITION INCLUDES; Books, Supplies, Lifetime access to online study guide, practice tests, JOB placement assistance and Completion Certificate. Approved in all States.Ā
