I understand all reasonable safety precautions will be taken at all times by First Christian Church of Brazil, IN (FCC) and its agents during their events and activities. I understand the possibility of unforeseen hazards and know the inherent risk. I agree not to hold FCC, its leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form.

I understand further that volunteers not in any manner considered an employee of FCC. I also release FCC and their leadership from any liability for any damages, whatever their nature, which may result during a Kids Ministry-sponsored event or activity.

For the protection of the children in the church, FCC is required to inquire of its staff and volunteers whether or not they have ever been convicted of a crime related to children. We run background checks to ensure no one has done any of the following: aggravated murder, murder, voluntary manslaughter, involuntary manslaughter, felonious assault, aggravated assault, assault, aggravated menacing, abuse or neglect of a child, kidnapping, abduction, child stealing, criminal child enticement, rape, sexual battery, corruption of a minor, gross sexual imposition, importuning, voyeurism, public indecency, felonious, sexual penetration, compelling prostitution, promoting prostitution, procuring prostitution, disseminating matter harmful to juveniles, pandering obscenity, pandering obscenity involving a minor, pandering sexually-oriented matter involving a minor, illegal use of a minor in nudity-oriented material or performance, endangering children, contributing to the delinquency of children, carrying concealed weapons, improperly discharging a firearm at or into a school or objects in or adulterating food or confection.

I agree to abide by the health policy posted by the Kids' Ministry at FCC. Should my child become ill or suffer an accident during an FCC event or activity, I agree that I shall be responsible for any hospital and medical charges that may accrue.

I permit photographs in which I and my kid(s), who are listed on this paper, appear to be used for publications and public relations activities by FCC. This may include use in print and electronic media, social media, including the internet.
