This form will be used to make sure we can keep your pet and others as comfortable as possible while in our daycare. If you have a puppy that has never been introduced to these circumstances, we recommend taking him/her to a dog-friendly area to see how he/she reacts to give you more information. It is our goal at Blessed Paws to make sure we are providing a fun and safe environment. We take responsibility of caring for your pets very seriously. We ask that you be as truthful as possible so we can best assess and make the best plan for you and your dog.
