Silver Lake Elementary's Soccer Club Sign Up Form is no longer accepting responses. If you completed this form before 4:30pm on April 13, 2023, your child has already been placed on a team and we will see them at Spring Soccer Club's first practice on April 19th, 3:30 - 4:30pm. If you missed this deadline, we welcome you to sign up your child for Fall Soccer Club in September 2023. Go Mustangs!
