This form is to be completed by a coach or team manager to withdraw a swimmer from their events. Information will automatically be sent to the Gala IT team. We are using this form for advance and on-the day withdrawals.  Withdrawals must not be sent by swimmers (you need to talk to your team manager or coach please!)
