個人情報の取扱いについて

本申込における，個人情報の取り扱いについて，以下のように定めます。
 (1) 個人情報の利用目的
・申込情報を確認し，適切な情報を提供するため
・当日の運営に係る事務または業務を遂行するため
(2) 保有する個人情報の提供
・本調査で管理する個人情報は，次に掲げる場合を除いて第三者に提供することはありません。
・本人の同意があるとき，又は本人に提供するとき
・国の機関，他の公的機関等において当該機関の業務遂行に必要な限度の提供に係る個人情報を利用し，かつ，当該個人情報を利用することに相当な理由があるとき
・専ら統計の作成又は学術研究の目的のために保有個人情報を提供するとき
(3) 個人情報の開示，訂正等について
本調査の担当事務は，利用者本人からの自己情報の開示，訂正の求めがあった場合は確実に対応します。

Privacy Policy
Graduate School of Life Science have established personal information protection policies on handling of personal information in this application as follows.

 (1) Purpose of personal information use
Graduate School of Life Science shall use personal information for the purposes of:
・Confirming personal information and providing appropriate information;
・Carrying out administrative work and operational responsibilities to ensure the smooth running of the day.

 (2) Provision of personal information
Graduate School of Life Science does not provide or disclose personal information in our system to a third party except the following:
・When the consent of the person concerned is obtained, or the person concerned has his/her own personal information disclosed to himself/herself;
・When using the minimum required personal information provided with for the execution of operation by public authorities or other public agencies, and using it for adequate cause besides; and
・When making use of personal information for statistical or academic research purposes.

 (3) Disclosure and amendment of personal information
When an applicant makes a request for disclosure or amendment of his/her own personal information, Graduate School of Life Science will ensure an appropriate response to him/her and effect the change promptly.
