Switching to Hybrid Class
Please complete this form if you would like to switch your child from an all-remote to a hybrid class.  Children in hybrid classes attend school in person 2-3 days a week. If you have more than one child you are switching to hybrid, you must complete a separate form for each child.  Note that although the DOE has extended the deadline for applying using their form to November 15, it would be helpful to have this form completed earlier if possible.  YOU WILL STILL HAVE TO COMPLETE THE DOE FORM AND WE WILL SEND YOU A LINK TO IT WHEN IT BECOMES AVAILABLE.
