Please submit this form to order Muir Cross Country gear. Note that the Red Cross Country shirt is MANDATORY.

Upon completing this form, be sure to bring in cash or a check to pay for your gear. Checks can be made payable to Muir Middle School. ALL PAYMENTS ARE TO BE GIVEN TO COACH GUSTAFSON!

There are sample t-shirt and sweatshirt sizes available at practices. There are no refunds or exchanges for ordering the incorrect size for your child.

If you fill out two t-shirt order forms, you will need to pay for two shirts (this is the form we use to order the gear). Please contact me with any changes I need to make to the order or submitted incorrectly.
