1) I understand that my child will be participating in volunteer activities at Human Robotics - Non-Profit.
2)  I understand that Human Robotics - Non-Profit is a non-profit organization that provides services to the community.
3)  I understand that my child will be supervised by adult volunteers at all times.
4)  I understand that my child will be responsible for following all rules and regulations of Human Robotics - Non-Profit.
5)  I understand that Human Robotics - Non-Profit is not responsible for any injuries or accidents that occur to my child while participating in volunteer activities.
6)  I understand that I am responsible for providing transportation to and from volunteer activities for my child.
7)  I understand that I am responsible for providing first aid and medical care for my child while participating in volunteer activities.

8) I grant permission for Hubotics to use photos or videos of my child taken during the event.
