Allergen Waiver - 2023/2024 School Year
Nut & Food Allergy Waiver
This agreement has been created to serve as a waiver between Virtuous Kitchen Inc. at 739 King St. E. Cambridge ON N3H 3N8 and the legal guardians of all the children attending and participating in the school lunch program offered at participating schools. Some of the children have been identified as having severe allergic reactions to certain major allergen food items such as Shellfish, Sesame, Dairy, Cashews, Tree Nuts, Eggs, Soy,  Etc.

Please be aware that at Virtuous Kitchen Inc., as indicated on our menu, we will be providing some of the major allergens to the other children such as eggs, fish, soy, dairy, etc. 

Please note: we do not use peanuts, nuts, sesame in our kitchen however some items such as premade breads, sauces or other premade items may include statements such as "may contain" sesame, nuts, dairy, etc or other allergenic ingredients. We cannot be responsible for these ingredients and if you are unsure or your child has had reactions to these types of foods in the past, I would advise caution. 

This waiver form must be completed prior to the start of your child participating in our school lunch program.  We thank you and look forward to a great school year!  By submitting your registration form online, you hereby acknowledge that you are aware that Virtuous Kitchen Inc. cannot guarantee a completely allergen-free atmosphere in our school food program and you have chosen to allow your child(ren) to participate fully.

By completing this form and providing your electronic signature below, you understand the inherent risks associated with participating in the school lunch program at your child's school and agree to have your child(ren) participate.  By completing this form, you identify that you have provided accurate information regarding your child's food allergies and preferences and will not hold Virtuous Kitchen Inc., or any of its employees, liable if a food-related incident were to occur.

Cancellation Policy
Sick Days/ Bad Weather days/School Closures

Due to the nature of running a food business and the need to plan and prep ahead for all orders, we will not be able to offer refunds for last minute cancellations of less than 24 hours notice. We will donate all possible food items to the homeless shelter in Cambridge if we can safely do so. We will gladly apply a 50% discount on missed lunches to your future orders. 

If your child is ill and you need to cancel your order, please notify Virtue directly via Text at 519-830-4244. Please indicate your child's name and how long they will be absent. For all cancellations due to illness, we will apply a 50 %  discount to your account towards the next enrollment period. 

For prolonged bad weather days or school closures of more than 1 week, we will apply a full credit to your account for the next enrollment period. Thank you for your continued support.
