RLC Youth Ministry Health and Participation Forms
Welcome to another year of Youth Ministry at Resurrection Lutheran Church.  I am so excited about the full calendar of events and happenings this year and all of the possibilities not even realized yet.  ALL Middle School and High School students (6th grade - 12th grade) who participate in even one activity (ie. service event, Sunday School, youth group, STUDY group, retreats, camp, etc.) need to have these forms filled out in order to participate.  Forms only need to be filled out one time and are good for the entire school year.  These forms will be in effect until August 1st, 2023.

I would love to have all of these forms filled out by September 1st.  I will have paper copies available upon request if you would prefer to submit the information that way.  All information is kept confidential.  I am so excited and look forward to working with all of you!
