Please use this form to indicate if you will be attending the Memorial Service for Wave Starnes in-person or online.  Our current capacity limits require that we limit the number of people attending in person to 175.

Please note that if you are attending in-person, Dumbarton UMC has the following health and safety protocols in place:
1) face coverings are required inside the building at all times;
2) please maintain physical distance of 6' or more whenever possible inside the building;
3) full vaccination against Covid-19 is not required, but strongly recommended;
4) name, email and phone number will be collected by the church in order to conduct contact tracing, if needed.
