Come Play Kickball
Join the North Whidbey Pool, Park, and Recreation District this summer playing kickball. This is an hour of fun for your child and their friends of the same age. NWPRRD will be on-site to keep score, officiate and help players. Teams will be changed up daily. Just some fun in the sun!

I give my permission for the child listed above to participate in North Whidbey Pool, Park, and Recreation (hereafter NWPPRD) programs and activities and use its facilities and equipment. I do hereby for myself, my heirs, executors, and administrators, waive, release, and forever discharge NWPPRD, its’ employees, and agents from any causes of action, claims, liabilities, or demands of any nature whatsoever, including but not limited to a claim of negligence, for personal injury, bodily injury, property damage, death or accident of any kind, arising out of or in any way relating to their participation in activities or programs, and/or use of equipment or facilities in activities, programs or facilities owned and/or operated by NWPPRD, Oak Harbor School District or the City of Oak Harbor whether by the negligence of NWPPRD or otherwise. I further agree not to sue and agree to indemnify and hold harmless NWPRRD and its partners from any and all causes of action, claims, demands, losses, or costs, including attorneys’ fees, of any nature whatsoever arising out of or in any way relating to my child’s participation in activities or programs, and/or use of equipment or facilities. *
I hereby consent to and permit emergency treatment in the event of illness or injury while using the equipment or facilities and/or while participating in the activities and programs of the North Whidbey Pool, Park, and Recreation District and its partners, such as the City of Oak Harbor and or the Oak Harbor School District. *
I hereby give the North Whidbey Pool, Park, and Recreation District permission to photograph and/or videotape my child. The sole purpose of these photos and/or videos is for publication, advertisement, and exhibition of services by the North Whidbey Pool, Park, and Recreation District. *

I further certify that I am at least eighteen years of age, or, if under eighteen years of age, my parent or legal guardian has signed on my behalf and that I am otherwise legally competent to sign this agreement.
