This is to register your canine(s) with WiggleWaggles to use our services. You can register up to 3 canines, if you only have 1 or 2 then you can skip the duplicate questions. Please make sure you fill in all information relating to your dog on walks, even if if I am not walking/feeding them. This just gives me a better understanding of your dog(s) in general.
If you are updating or resigning for a new year, then please update any new information, then sign and date on the last page. If you do not have an up to date form for the current year, WiggleWaggles cannot accept any new bookings.
