I hereby consent my photographs taken and videos recorded during any Life Point activities, events and outings to be displayed/uploaded on Life Point pamphlet, website and multimedia for publicity and educational purposes.

Consent To Use Personal Data

By giving my consent, Life Point may use my personal data for analysis and promoting activities in connection with Life Point’s mission.

My personal data shall be used by Life Point to verify my identity and to allow my continual participation in the programmes organise by Life Point. In undertaking the foregoing activities, Life Point may need to share my personal data with its service providers or partners for these stated purposes. I hereby consent & authorise Life Point to collect, use, provide or disclose my personal data for the purpose stated above.

Council for Third Age，活跃乐龄理事会，简称C3A，成立于2007年5月， 本理事会提倡活跃乐龄的理念，让我国的乐龄人士过积极与正面的晚年生活。活跃乐龄理事会把终生学习、活跃乐龄和强化乐龄义工社会理念三大领域作为重任，理事会在乐龄工作上起了引领带动的作用。活跃乐龄理事会(C3A)被委任为全国乐龄学苑 (National Silver Academy, NSA) 的行政机构。

The Council for Third Age (C3A), set up in May 2007, is an agency which promotes active ageing in Singapore through public education, outreach and partnerships. As an umbrella body in the active ageing landscape, with its focus on senior learning and volunteerism, as well as positive ageing, C3A works with and through partners to help third agers age well. C3A is appointed to administer the National Silver Academy (NSA).

Consent for Use of Personal Data
By submitting this form, I consent to my personal data being collected, disclosed to, and used by C3A and its relevant partners, for purposes that shall include but not limited to the following: course administration, keeping me informed of relevant information on programmes, research and other publicity relating to active ageing, as well as for statistical studies, comparison, research and/or any other related use. Do note that photographs and videos may be taken during the course for publicity purposes.
