Please fill this out if you are planning to have items that will be sold at the Yard Sale. If you have 5 or more items, you will have to manage your own booth and sales at the yard sale. FMoPA will be managing sales for people that are bringing less than 5 items, so section 2 of the form will be for getting information about the items you are bringing.

If you are managing your own booth (you have 5 or more items you are selling), you are clicking agree to acknowledge that you will be responsible for your sales during the yard sale. FMoPA will not provide cash change, point of sales system, and other sales services to vendors, so you are expected to be prepared to accept the payment methods of your preference.
