You will need to bring the following items with you on the first day of training. You may print this page if needed.

1.Valid Drivers License
2.Valid Social Security Card
3.Proof of Automobile Liability Insurance-this must be the declaration page that shows the limits of liability and the dates of coverage. Upon renewal, this must be turned in to the Human Resource Director. This is part of your responsibility as an employee.
4.Registration card for the car you will be driving while transporting clients. Upon renewal, this must be turned in to the Human Resources Director. This is part of your responsibility as an employee.
5.G.E.D. OR High School official transcripts are required.
If you are a graduate from a Surry County High School you must go to https://www.surry.k12.nc.us/ click the parents/students tab then click on Transcripts, Records Request, and Education Verifications and follow instructions to request an official transcript. Please note that the cost is $10.00 and is the responsibility of the employee and is not reimbursable. Please ask that the transcript be mailed to Behavioral Services Inc, 2342 South Main St., Mt. Airy, NC 27030. Attention Pam Padgett.

If you are a graduate of Mt. If you need to obtain a copy of a GED Transcript:
It must be completed through www.DiplomaSender.com. Please request that the official transcript be mailed to Behavioral Services Inc., 2342 South Main St., Mt. Airy, NC 27030. Attention: Pam Padgett. The website will offer an option to email you a copy. DO NOT SELECT AN EMAIL COPY!! IT HAS TO BE A PAPER COPY WITH A RAISED SEAL. Note: There is a cost which is the responsibility of the employee and is not reimbursable.

6. If the employee is a C.N.A., we ask for a copy of your license from the North Carolina Registry.

If you do not have the above items with you when you arrive for training, you will be asked to reschedule. You must have these items to participate in training.

On your first day of training, you will be required to have a TB Test and a Pre-Employment Drug Screening. This will be completed at the Occupational Health Office at Northern Regional Hospital in Mt. Airy, NC. The cost is $30.00. The employee is required to pay for this the day of the screening/test but will be reimbursed after 60 days of employment if a receipt is turned in the Human Resource Department.
