Working at ADF

This form has been put together in order to enable smooth and safe working environment at ADF. When working at ADF please male sure that maximum capacity in different spaces is not exceeded.  We must keep a log of space use so we can trace the users if necessary. All the spaces are marked with red paper sheets (picture below) indicating the max capacity.

By filling out this simple form and by following the max space capacity, we should be all good to go! The data is  used to reach you out if positive COVID cases occur at ADF.

Data usage: Your information will only be used to contact you if we have a positive COVID case at DF, and all of your information is erased every 90 days.
