This application is required to be considered eligible for adoption of a NBSTR dog.
**Before you fill out this application, be sure you have read and understand the application guidelines. Applications that do not contain all of the requested information or meet our guidelines shall be discarded.

Please note we are now asking for a $10 Application Donation

We give priority to those applicants who have a completed application on file. The completion of an application does not obligate you to adopt a specific dog. We keep applications on file for six months.

Our adoption process consists of:
 
Completion of an application,
Reference check (this may take up to two weeks)
Review by the foster home to find the best fit for the dog.  
Home Visit Completion by foster home or another NBSTR volunteer.

What can I do to make the process go quicker?  

1. Make sure that your application has been completed fully
2. Let your references know that we will be contacting them.  
        ****Your vet WILL NOT release information to us without prior approval from you.****
3. PLEASE DO NOT APPLY IF YOUR CATS/DOGS HAVE NOT BEEN SPAYED OR NEUTERED.
4. PLEASE DO NOT APPLY IF YOUR DOGS ARE NOT UP TO DATE ON VACCINATIONS.  
5. PLEASE DO NOT APPLY IF YOUR DOG/S HAVE NOT BEEN TESTED FOR HEARTWORM YEARLY OR ARE NOT ON HEARTWORM PREVENTATIVE.

In special circumstances, we will approve if a Veterinarian had a Medical reason not to spay/neuter, vaccinate, or heartworm/preventative.  If this is the case, please explain in the application and we will still consider you for approval.
 
Final approval for the adoption rests with the foster home.  We always look for the best possible match for the dog.

Your submission of this application implies your consent for a NBSTR volunteer to contact you and all references that you have listed. Your submission of this application also shall indicate your sincerity of intention regarding adopting a NBSTR dog, and that all information you provide in this application is complete and accurate. All information provided in this application must belong to the person who will be adopting the dog.
