To enroll in this study as a Trainer, please complete the following Trainer Participation Intake form.  You will be listed on the study website as an approved trainer on the "Find a Trainer" webpage.

Providers will use their existing flow of clientele to populate this study. There will be a one-page printable brochure to advertise this study to attract and inform new study participants. Prospective participants who do not have a trainer already will be matched with one closest to them, or to one who offers NeurOptimal® rental systems. Information collected in this study will not be shared with trainers until the end of the project, after the data has been analyzed. They may assist the participant with submitting post-session surveys by providing a tablet, computer or other internet-enabled device. It is not a requirement for the trainer to fill out the post-session survey for the client, however they may do so in an interview-style Q&A if it is needed.

It is not our intention to accept printed versions of the post-session survey however, if a trainer has the desire to use this method, they may enter the data into the post-session survey portal themselves for their client, then they may do so using their own printed resources. Participating trainers will follow confidentiality and ethics practices to ensure proper handling of client information and ethical treatment of all participants.
