Agreement:
I, parent of the Silvies River Charter School student noted below, certify that I am in possession of the (1) laptop and/or chromebook computer or MiFi noted, hereafter referred to as 'Device'.  I understand that this device has been provided solely for my use with my school work and school communication only. I will not use this device for personal reasons, such as: social networking, text messaging, games, videos or downloading any program without the approval from my Silvies River Charter School teacher or for any type of cyber bullying. Failure to follow the above guidelines will result in my loss of using SRCS's device and to repay any usage costs associated with inappropriate use. I accept that I am responsible for the repair costs for damage to the device beyond that caused by normal use. I have inspected the device and described any apparent pre existing damage to it on the attached condition form. I agree to inform the Silvies River Charter School technical support department if I become aware of any problems with the proper functioning of the device. I understand that it is my responsibility to return the device to the Silvies River Charter School within five (5) days of my withdrawing from the school. I also understand that the failure to return the device or pay the damage to it shall result in a report to law enforcement for theft and/or filing a claim with Small Claims Court to recover the device or its full replacement value. I further agree that any dispute arising from this contract will be resolved according to the laws of the State of Oregon and promise to pay all reasonable collection or legal expenses incurred by the school should collection or legal action become necessary to recover the computer. I agree that this document constitutes my entire agreement with the Silvies River Charter School and cannot be modified by verbal agreement. I further agree that this contract and all provisions therein apply.
