There are three parts to this registration process. Please complete all three in order to ensure your enrolment in the program.

Which program are you joining? (if you have multiple children, please complete a separate form for each child) *

ONLINE Junior (ages 8-12, Wednesdays, 10am to 11am), $500 total

ONLINE Senior (ages 13 - 19, Wednesdays, 11am to 1pm), $700 total

LIVE Junior Program (8 - 12, Mondays 10am - 3pm), $1100 total

LIVE Intermediate Program (ages 11 - 15, Mon & Fri, 10am - 3pm), $2100 total

LIVE Senior Program (ages 13 - 21, Mon & Fri, 10am - 3pm), $2100 total

2. AGREEMENTS

Photo and Film Release

I agree to have photos or videos taken that include my minor child and that may be used for promotional purposes

Parent Participation

I understand that parents might be asked to volunteer before or during shows

I understand that parents of younger children may be required to remain on-site to support a smooth transition into the program

Liability

The TES Theatre Company staff and volunteers cannot be held liable for personal injury or loss, theft, or damage of personal property

3. PAYMENT

Payment can be made in three ways:

You can pay the year's total now, all at once.
OR you can pay half up front, and the remaining half on January 1st.
OR you can pay 10 equal monthly payments, beginning now, and then on the first of each month for 10 months.

Online Junior: $500 total
Online Senior: $700 total
Live Junior: $1100 total
Live Intermediate: $2100 total
