In order to develop a meaningful emergency plan for all who attend UUCM, we ask that every attendee complete this questionnaire and return it to designated baskets in the Narthex. Some who do not appear to have mobility challenges might very well need assistance under emergency circumstances for other physical, cognitive or psychological reasons.  Please take a few minutes to complete this questionnaire so that the UUCM Emergency Preparedness Safety Team can take the needs of each person into account.   If you have any questions or do not feel comfortable completing this survey, please contact Mary Moriarty  at marymoe.55@gmail.com.
