Thank you for registering to Rise Up's Summer Arts Academy! This form is the first step in getting your child registered for the camp!

Camp dates are: 7:30 AM- 4:00PM, Monday - Friday July 10-July 21, 2023 at Edgewater Elementary.

In order to register your child/ children you will need to fill out this registration form. Once completed, you will be directed to a link to check-out on our secure pay portal. Your child's spot in the camp will be reserved upon receipt of payment.

Please have some documents ready for the following registration form. You will need: Your child's information (including date of birth, legal name, preferred name, age, dietary restrictions, and medical conditions), Your information (including legal name, preferred name, address, valid email address, and phone number).
