I acknowledge that I am fully aware of the potential dangers of participation in any sport and I fully understand and accept that participation in football and cheerleading may result in SERIOUS INJURIES, PARALYSIS, PERMANENT DISABILITY, AND/OR DEATH. Furthermore, I fully acknowledge and understand that protective equipment does not prevent all participant injuries, and therefore I do hereby waive, release, absolve, indemnify, and agree to hold harmless EXCELSIOR SPRINGS FOOTBALL ASSOCIATION, the local league, and any organizers, sponsors, supervisors, participants, and persons transporting the above-named participant to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause. PARENTS, GUARDIANS, OR ATHLETES WHO MAY NOT WISH TO ACCEPT THE RISKS DESCRIBED IN THIS WARNING SHOULD NOT SIGN THIS FORM. YOUTH ATHLETES MAY NOT PARTICIPATE IN AN ESFA-SPONSORED SPORT WITHOUT THE SIGNATURE OF A PARENT OR GUARDIAN.

I acknowledge and accept that Football and cheerleading require a high level of dedication, hard work, and commitment. I agree to hold myself and my child accountable and attend all practices and games possible. I will communicate any absence with my child’s head coach.

I acknowledge and accept that ESFA and the WCYFL are competitive leagues, and head coaches have no obligation to play a player. Playing time is not guaranteed and is determined by dedication, player behavior, team needs, available players, and skill level.

I agree to assume full responsibility for any equipment/uniforms loaned to the above-named participant, including inspecting my child’s equipment daily and notifying ESFA of any concerns. I agree to promptly return, upon request, the uniform and other equipment issued to the above-named participant in as good condition as when received except for normal wear and tear. If I fail to adhere to this policy, I will be responsible for the replacement cost of such equipment, and acknowledge that ESFA will pursue this replacement in a court of law.

I agree to furnish an authentic certified copy of the birth certificate of the above-named participant to the local ESFA officials.

I acknowledge and accept that pictures and images of my child may be taken and authorize the use of my child’s picture or likeness to be posted publicly. I understand it is my responsibility to make ESFA representatives aware of any Custody or personal protection issues that require my child’s picture to not be publicly posted.

I acknowledge and accept that my child is to conduct themselves as a representative of ESFA and display leadership and respect outside of ESFA activities. I understand that frequent issues with school administration or with my child’s peers could result in discipline being enforced in practice or games, or being removed from the team.

I agree to pick my child up from practice and games promptly or communicate any emergencies with my Child’s coach or ESFA Representative. If I am more than 15 minutes late and have not made contact with an ESFA representative, I acknowledge that the local authorities will be notified.

I acknowledge it is my responsibility to provide transportation, but if needed give permission for my child to receive a ride to and from the activities by a licensed driver with proof of insurance.

I hereby grant my permission for any emergency/dental treatment and or first aid to be administered to my child/ participant, including authorizing any medical treatment facility/hospital to administer emergency treatment for any illness/injury/accident resulting from participation in any ESFA activities.

I, the parent guardian of the above-named participant hereby acknowledge that my child is in good general health and I give my approval for my child to participate in any ESFA activities.
