This form is for high school students who have completed a formal program of study over the summer and wish to take an exam to demonstrate mastery of the course. The exam will be offered on August 24, 2022 at 1:00 at DHS in room B115.

**If a student has completed informal work and wishes to sit for a placement exam, we will make arrangements during the first week of school with the students' math teacher.

The placement exam will be given on Wednesday, August 24 at 1:00. The location is the DHS Math Learning Connections Room.
