3. Name of your organization *

If you are category A in the previous item, write an abbreviation of your NGO (e.g., GIVE, GREAT, VPV). If you are B, write an abbreviation of your network (e.g., ALLIANCE, CCIVS, SCI). If you are C, write full name of your organization. If you are D, write "none" unless you belong to any organization which may be interested to cooperate with NVDA!

4. Your role/ position in the organization

E.g., Active member, Project Coordinator, Board (EC) member, Executive Director, Local host

5. Your nationality/ residential country *

If they are same, just write one (e.g., Thailand). If different, write Nationality/ Residence like Indonesian/ Vietnam.

6. The programs you will join *

Choose all the programs you would like to join! Please note that Programs 9 is open to only NVDA members, Program 1, 2, 5, 6, 10 are open to also Sister networks, Program 11 is open to also the guests from Governments/ Companies/ Universities. Program 3, 4, 7, 8, 11, 12 are open to everybody!
