Streaming Video Request Form
This form should be filled out  by Clark College staff and faculty members requesting one of the following:
1. streaming video purchase for college-wide access
2. streaming rights purchase for a digitization of a film owned by Clark College Libraries in DVD format
NOTE: Please make only one request per form. DO NOT use this form for more than one request.

Note that NEW streaming video platforms also require new licensing agreements to be signed and reviewed by Clark College Risk Management Department.  The negotiation of acceptable terms may take 2-3 months as well as meet the criteria for Clark's accessibility requirements.
