Are you ready to dive into some self-discovery and reflect on your current context? This questionnaire covers themes related to the 5 Principles of Exercise and Weight Management; it's like a friendly chat to help you figure out where you're at and what's next on your wellness journey. Each question is a little clue on what you might need to think about, and more importantly, the awesome moves you're gonna make moving forward.

Grab a pen and paper for those extra thoughts that might pop up along the way; this questionnaire is likely to make you think outside the questions asked.
