Your participation is voluntary and your answers can be anonymous. It is completely optional for you to write your name at the bottom of the survey. The answers from all surveys will be reviewed by our administration team, and your individual answers will be confidential.  Staff members will not have access to see any of your answers so we encourage you to be as honest as possible, so we can better serve your needs.  Even if you give us permission to use your answers for marketing/testimonial purposes your name will not be associated with your comment.
