Device Request Form

Please complete this form if you would like to request a device for your child. Students in grades K-2 will receive an ipad and students in grades 3-5 will receive a Chromebook. Devices will be available for you to pick up, along with school supplies, on the following schedule by grade level. If you have children in more than one grade, you can pick up devices and supplies for all your children at one time.
