Baptism Questionnaire
Thinking of getting baptized?  We couldn't be happier for you and your decision to show this profession of faith publicly.  Below are a few questions we are asking you to respond to about how you came to this decision.  (If you're a child, feel free to have your parents help you with this form.)

Once we have received your answers, someone from the church office will reach out to you to make an appointment to meet with you and set a date for your baptism.

For your baptism, we give you a special baptism t-shirt to wear.  What size t-shirt do you wear?  We will do our best to accommodate closest to your size. *
Once you have met with one of the pastors, you will be asked if you have a date you'd like to be baptized.  If you know that date when family/friends can come, what is that date and we will do our best to fit that in to the Worship Service that day.  If you don't have a specific date, just say "Don't know". *
