Camp in The City: Authorized Pick Up Form
Please complete the below form for those authorized to pick up your camper. This form will be used to verify the identity of anyone who picks up your camper without a pick-up pass in hand. We will also use this information if we cannot reach you in the event of an emergency. Parents will receive a different color pick-up pass each Monday; only the color of the week will be honored. Thank you for your understanding, as this process ensures all of our campers are safe.

I understand that persons I list are authorized to pick up my camper from Camp. I also understand that any person who presents the current pick up pass for my child will be authorized to pick them up. Pick Up passes should only be given to persons authorized to pick up my child. Lost or missing pick up passes should be reported to camp staff immediately. *
