Aquin Senior High Football Camp 2019
Camp will run Tuesdays-Thursdays in July from 3:30-5:30pm at Aquin High School.  All campers receive a t-shirt and shorts.  Please submit one form per camper.  Payments may be submitted to either Aquin Office--$50 per camper.  Campus address for payments is 1419 South Galena in Freeport.

I hereby request that you enroll my child in the 2019 Aquin Football Camp.  I hereby release the Aquin Catholic Schools and its employees and volunteers of all claims on account of any injuries that may be sustained by my child while attending the Football Camp.  Additionally, I agree to indemnify the Aquin Catholic Schools’ employees and volunteers for any claim that may hereafter be presented by my child as a result of such injuries.
