Survey: Braille Music Literacy
This survey is part of a research project which is seeking to identify possible correlations between braille music notation literacy and satisfaction in music-related activities and employment. Data from this project will help teachers and others who design music curricula to improve accessibility, equity, and opportunity of music programs, and better prepare music students for success in music careers and personal or community music activities.
Important Information about this Survey
You have been asked to participate in a research project conducted by Dr. Toby W. Rush from the Department of Music at the University of Dayton.

The purpose of the project is to identify possible correlations between braille music notation literacy, professional success, and quality of life.

You should read the information below, and ask questions about anything you do not understand, before deciding whether or not to participate.

- If you agree to participate, you will be asked to complete a brief, online survey. Answering the questions will take about 10 minutes.

- Your participation in this research is voluntary. You have the right not to answer any question and to stop participating at any time for any reason and without penalty.

- You will not be compensated for your participation.

- All of the information you disclose will be confidential. Data shared from this project will only be available in an aggregate form that ensures the anonymity of individual responses.

- Only the researcher will have access to your responses. Identifying information will not be collected, but the security of the computer you use and the security of data transfer between that computer and our data collection point cannot be guaranteed. Please consider this carefully when responding to these questions.

- You are ONLY eligible to participate if you are over the age of 18.
