24-25 Cheer Tryout Permission Forms
Thank you for your interest in our program.  Please complete the form below with needed contact information and permissions to participate in our events.  By completing this form, you are agreeing to abide by our cheer constitution (all rules and expectations) if you are selected for the team. 

This form is to be completed by a parent/guardian and athlete.  It is crucial that you as a family are in agreement about the time/money commitments aligned with being apart of this squad.

Please list tumbling skills (bhs and higher) that you can consistently throw on the HARD floor.  Tumbling skills are not required to make our Varsity team. Safety is our top priority. Please be sure to be honest in your assessment of skills.   Your athlete will only be allowed to perform the below listed tumbling skills at the tryout.

Please share your experience (position and level) in stunting.   Stunting will not be included as a portion of our tryout this season and is only required as a prerequisite for the competition team.

My daughter plans to attend the optional prep clinic held on March 13th. *
I give my student permission to attend the Elkhorn South Cheer prep-clinic to prepare for cheer tryouts. I understand that like any sport, there is a risk for injury. I understand that my daughter will be jumping, kicking, doing motions and dancing during the clinic. She will learn the fight song that will be performed at tryouts. She will NOT attempt any tumbling at this clinic. *

We, cheerleader and guardian, have read the rules and regulations of the Elkhorn South Cheerleading Squad.  We agree to follow all the rules and do the best job possible as a members and representatives of the squad.  If selected for this team, we will be available for all practices, fundraising, summer camps and/or clinics, performances and other activities as outlined in the constitution. We understand how the tryout process works and will adhere by rules set by coaching staff and constitution.
