PEER Ambassador Agreement & Profile

The Communities That Care Coalition brings together schools, families, youth, and the community to improve youth health and health equity.  The Coalition has initiatives focused on reducing youth substance use and other risky behaviors, on promoting nutrition and physical activity, and on addressing white supremacy culture and advancing racial equity.

The Communities That Care Coalition is working to increase opportunities for more community voice in our coalition, and we know that parent leaders offer unique perspectives, valuable feedback, and the ability to connect more deeply with community members most affected by the issues we are working to address.

We are striving to have a diverse group of parent leaders (called PEER Ambassadors where PEER stands for Parent Engagement, Enrichment, and Resources) who can provide insight representing different genders, incomes, geographic locations, races, ethnicities, health issues, and more.  With the help of PEER Ambassadors, the Communities That Care Coalition and our member organizations can improve the quality of services we provide and reach more deeply into the community, increasing our collective ability to improve youth health and health equity.

VALUES:

The Core Values of the PEER Ambassador Program, as determined by the PEER Ambassadors themselves, are:
*  Respect… As PEER Ambassadors we respect others for who they are and treat everyone with dignity
* Integrity… As PEER Ambassadors we choose transparency and choose to do the right thing in our communities.
* Confidentiality… As PEER Ambassadors we promise to maintain a level of trust and compassion to the people we support.
Responsibility…  As PEER Ambassadors we follow through on our tasks and seek help when needed.
*  Social Justice… As PEER Ambassadors we are dedicated to doing our part to improve social change for everyone in our communities.
*  Acceptance… As PEER Ambassadors we accept others in total; their differences, flaws, attributes, where they have been and where they are going.

I understand that I am a volunteer and I acknowledge that my participation as a PEER Ambassador does not create an employee/employer relationship with my Host Site.  I understand that PEER Ambassadors are  eligible to receive a stipend of up to $1,200 per year dependent upon active participation in the program. These stipend funds will be dispersed approximately monthly upon the successful completion of my participation, and subject to monthly review by my Host Site Supervisor.
As a PEER Ambassador, I will meet the terms as outlined for participants in this PEER Ambassador Agreement, and will let my Host Site Supervisor know if I am no longer able to meet these responsibilities.
