Comment / Request Card for Website
Hello! This Google Form is a simple way for the community to communicate with the school about the website. 

This comment / request card can be used to notify if a part of the website is not working properly, if you would like to see something added or changed about the website, or if you would like to have information shared via the website. 

All responses will be read but not all requests can be completed. The school greatly appreciates your feedback on the new website!

Is there any operating issues that you notice on the website? For example, pages are not loading, photos are not clear, articles are unable to be read, etc. 
Do you have any recommendation/ideas for how the template runs like anything added or changed? For example, activities calendar in a new place, ensure new photos every week for the image rotator, staff pages easier to find, etc. 
Is there any information you would like published on the website? For example, community events happening, school photos you have taken for the image rotator, to share student's accomplishments outside of school, etc. 
Are there any other comments you would like to share about the new template for website?
