REGISTRATION
To complete the registration,
1. Fill out the registration form
2. Send the appropriate payment
3. Submit a health form signed by the child's doctor by May 31st.

REGISTRATION IS NOT COMPLETED UNLESS WE HAVE RECEIVED A FULL TUITION and the HEALTH FORM IS SUBMITTED BEFORE THE DEADLINE.

CANCELLATION AND REFUND POLICY
We will refund the summer program tuition in the following way:
100% if a cancellation notice is given 4 weeks or more before the start of the program
75% if a cancellation notice is given 2 – 4 weeks before the start of the program
50% if a cancellation notice is given 1 – 2 weeks before the start of the program
No refund will be given if the cancellation notice is given less than 1 week before the program.

SNACKS AND LUNCH
Please pack snacks and a lunch for your child. Please do not include nuts.

DISMISSAL POLICY
1. A teacher may dismiss a student for a part or a whole class if a student’s behavior is disruptive for the educational process or in any way harmful to other students or teachers, as well as for violations of the studio rules.
2. The Studio reserves the right to expel a student from the summer program for behavior which is disruptive for the educational process or in any way harmful to other students or teachers, as well as for repeated violations of the studio rules. In this case, the Studio will refund the unused portion of the tuition.

SUNSCREEN POLICY
The students will play outside on most days. Please put a layer of sunscreen on your child in the morning.

PHOTOS
Studio of Engaging Math may use pictures of students for future public relations materials (web, print, etc.)

CONSENT
I, the parent or legal guarding of minor listed on this registration, hereby give my permission for my child or ward to participate in the Studio of Engaging Math summer program. I understand and accept all the rules, terms and conditions outlined in this registration form.

In case of a medical emergency and in the absence of an authorized parent or guardian, Studio of Engaging Math has my authority to transport my child to the nearest hospital and to secure for my child all necessary medical treatment, including anesthesia.

I give my child permission to participate in Studio of Engaging Math Summer Program trips and excursions, sports and swimming activities. I understand that public or private transportation may be used to get to museums and parks.

I hereby assume all risks of my child’s attendance at the program and my child’s participation in its activities. I release and agree to hold harmless Studio of Engaging Math, its employees and contractors from any actions, causes of action claims, demands, damages, costs, loss of services, expenses and compensation on account of any personal injuries or property damage which I may hereafter have as the parent or guardian of said minor.
