Please complete this form multiple times if you have multiple children attending Tanner for the 19-20 school year.  After submitting the form you will have the opportunity to select "submit another response".

This form provides us with detailed information on the transportation plan you will have in place for your child.  This information is important for the main office, classroom teachers and bus company to be aware of.  If this form is not completed and submitted prior to August 1st, bus transportation cannot be guaranteed for the first two weeks of school.  

District Transportation Policy:  The District shall provide transportation for students, residing within the District boundaries, under provisions of Wisconsin laws and regulations and as set forth in this transportation policy.  KASD 710:  All students living more than the distances listed from the school of their regular attendance area will be transported.  The distances are measured from door to door by the usually traveled route.  4-year-old and 5-year-old Kindergarten students will be transported door to door if they live over a half (.5) mile from school, grades 1-4 at bus stops if the student is more than one (1) mile from school, middle and high school student at bus stops if the student is more than two (2) miles from school.  
**Students Open Enrolled into the district are not guaranteed busing.  
**Home address and babysitter address must meet criteria noted above.
