The Merton Community School District appreciates all volunteers in our schools! Volunteers are important people and play an essential role in the educational process within our schools. By filling out this form, you are agreeing to the following:
- I have read the 353.1 School Volunteers Policy
- I shall agree to abide by all Board policies and District guidelines while on duty as a volunteer;
- I will be covered under the District's liability policy but the District cannot provide any type of health insurance to cover illness or accident incurred while serving as a volunteer, nor is the person eligible for workers compensation;
- In accepting the role of a volunteer, agrees to verification that a satisfactory background check may be conducted through appropriate State agencies or other applicable means;
- I must acknowledge in writing that they will respect the confidential nature of any pupil information, including academic, behavioral or other personal information they may gain access to as a result of serving as a volunteer;
- I must acknowledge in writing that they will not share any such information with any third party in valuation of District policies pertaining to pupil information
