This form is required by all registered athletes and coaches in order to receive your event bib(s). It must be completed one time only, between 72 and 48 hours prior to the event start date of March 10, 2021. Please complete this form inside this required window. Information will be monitored by staff, our Covid-19 Coordinator and Jury, and you may be contacted if there are questions.
NOTE: This form does not replace the Covid-19 Daily Symptom Check Google Form required by Soldier Hollow each race day morning prior to coming to the venue.
Anyone indicating experiencing COVID-19 symptoms should immediately be instructed to call the local COVID-19 hotline for testing instructions, or contact their personal healthcare provider, and club protocol should be followed to follow up with them. The local organizing committee is able to assist in scheduling testing locally for anyone experiencing symptoms during events at Soldier Hollow. Those experiencing symptoms should contact Judy Klautt via email at
