Let us know who your Confirmation Sponsor will be!

Each person who will be Confirmed in the Catholic Church chooses a sponsor for Confirmation.

Requirements to be a Confirmation Sponsor:
He or she must be at least 16 years old
He or she must be Confirmed in the Catholic Church
He or she must be a practicing Catholic (practicing means he or she is a member of the parish, attend Mass on Sundays, and has an active faith) who is a good example to you of what it means to be a follower of Christ and someone who will challenge you to follow Him in your own life
He or she cannot be a parent of the student, but may be (and is encouraged to be) the baptismal Godparent.

Please note that this form needs to be filled out by the teen preparing to receive the Sacrament of Confirmation and not by the parent or sponsor.
