3rd Nine Weeks Middle School Credit Recovery
This form is only for the Lamar Middle School students who have earned a failing 3rd 9 weeks grade in Language Arts or Math. If you would like your child to participate in recovering his/her 3rd nine weeks grade to a passing score you must complete this form prior to Tuesday, March 30, 2021.

Please have the Form completed and submitted prior to Tuesday, March 30, 2021 for our attendance records.

My student agrees to attend 4th 9 weeks on site as part of the requirements of this recovery program with no more than three absences excused by parent notes.

My student agrees to attend and be on task a minimum of 14 days of afterschool tutoring during the 4th 9 weeks with no discipline issues.
My student agrees to complete 18 assignments with a passing score for each course he/she is trying to recover.
