UPDATE: have added a N/A to most multi choice. You don't need to hit "N/A" for all items not received, it is only if you accidentally enter an item you didn't get.

Please note that it is extremely important that the data entry is consistent. I.e. please do not only enter GOOD data. We need the good, the bad and the ugly!!
