This form is an Expression Of Interest (EOI) form and waiver and release to participate in various activities at the A-League Grand Final Party on Friday, June 2 2023 at the Entertainment Quarter in Moore Park, Sydney [the Event]. Submission of an EOI does not confirm participation. If confirmed as a participant, further consents may be required and, if so, will be communicated at a later date.

By  participating in the Event/s, I consent to being photographed, filmed, and/or recorded by Australian Professional Leagues Trust (APL), its broadcast partners or other persons authorised by APL  ("APL Partners") and for any such images, photos, recordings to be stored, modified, edited and used by APL or APL Partners for any commercial, marketing and promotional purpose related to the Event including but not limited to post-event fan engagement, social media or future event promotions.

I [Full name] acknowledge the Participant is voluntarily participating in the Event with knowledge of the potential dangers and/or risks involved, and agree to assume any and all dangers and/or risks of bodily injury or death, whether those dangers and/or risks are known or unknown.  I [Full name] warrant that I am physically fit have no known, undisclosed medical condition which would prevent me from participating in the Event.

In consideration of being allowed to participate in the Event we, the Participants, individually and jointly hereby agree to forever release and hold harmless the Australian Professional League, its directors and employees in the event of injury to any Participant. 

I have carefully read and fully understand this waiver and release and agree to release APL from any liability for any injury or other losses we incur, including APL  acts of negligence or omissions to the fullest extent permitted by law. We also acknowledge and agree that we are signing this waiver of our own free will.

By consenting to this release and waiver form I:

(a) expressly state that I have read this document and fully understand and accept its contents;

(b) warrant that all of the information contained in this application is true and correct; and

(c) acknowledge that the APL has relied on the accuracy of this information in allowing me to participate in the Event.

(d) I verify that the dangers and/or risks of participating in the Event and the significance of this release and waiver have been explained to the Participant and that the Participant understood them.
