Personal Communication Devices & Internet Use Agreement - Student
As a parent/guardian, I/we have read and do understand the school district’s policy relating to the use of Personal Communication Devices & Internet Usage in School in the Parent/Student Handbook (Sections 7 & 8 in the ISD511 Parent/Student Handbook).  By checking below, I understand that if my child/children possess a Personal Communication Device (cell phone, laptop, Chromebook, etc.) at school, they are subject to the limitations of this and other district policies.  I further understand that any unauthorized use of these devices will result in confiscation of such device and other disciplinary measure deemed appropriate.

As a student, I have read and do understand the school district’s policy relating to the use of Personal Communication Devices & Internet Usage in School in the Parent/Student Handbook.   I understand if a violation is committed with a Personal Communication Device or Internet Usage that loss of class credit may occur, as well as access privileges may be revoked and school disciplinary action and/or appropriate legal action may be taken.
