This electronic waiver must be completed to attend any House of Paws Community Pack Walk. It must be completed prior to arriving on site to participate in the Pack Walk. Failure to complete this waiver will result in not being able to participate in the event.

House of Paws Mandatory Event Waiver Form

I wish to attend the House of Paws pet friendly event with my pet(s). I understand that this electronic waiver is mandatory to gain admittance to the event. In acknowledging so, I hereby agree to the following terms and waiver:

Rules and Eligibility Requirements:
• All pets must be on a leash not exceeding 6 feet in length and all pets must be in a suitable
harness/containment. You must have full control of your pet(s) at all times.
• All persons participating in the event who will be handling any of the dogs, must be 18 years of age or older
• All pets must either be fully vaccinated or have a current titer test proving immunity to communicable diseases.
• No retractable leashes or flexi leashes are permitted.
• No prong collars are permitted.
• Owners must clean up after their pet(s).
• Owners must report any incidents to event staff and adhere to the rules and must leave if asked to leave by any
event staff.
• Your pet must be legal to own in Regina and conform to all city by-laws.
• Each adult must fill out and sign this waiver form below, which confirms all previously stated information.

Indemnification: I am aware that at an event with animals, there is a certain inherent risk involved because I am attending with my pet and/along with other attendees' pets. In consideration for being allowed to attend this event, I acknowledge and voluntarily accept and assume full responsibility for any risks to me, my children and / or my pet(s) and further agree that I waive and release any and all claims that I may have against House of Paws Pet Boutique Inc. and/or the hosting location, its organizations, directors, officials, sponsors, event facility, any and all employees and volunteers of the aforementioned organizations and agree not to sue for losses, damages, expenses or fees which arise out of or result from, or are related to in any manner my attendance or participation in this event or future events associated with this event.

BY ELECTRONICALLY SIGNING THIS WAIER:
• You certify and agree to all terms as well as adhering to all event rules
• You take full responsibility for your pet and will be in full control of them at all times
• You confirm that all information contained in this electronic waiver form is true and accurate
• You confirm that your pet is not aggressive, not in heat, and has not had an incident of biting or shown aggression in the past and has never caused bodily or property damage.

• YOU UNDERSTAND AND AGREE THIS WAIVER SHALL BE BINDING AND EACH PARTICIPANT AND THEIR RESPECTIVE HEIRS, LEGAL REPRESENTATIVES, SUCCESSORS, AND ASIGNS SHALL INURE TO THE BENEFIT OF THE EVENT ORGANIZERS.
