Use this form to report a pre-planned absence for final exam days.  This form can be submitted by a student, parent, or teacher.  IMPORTANT:  Students who are approved for final exam make-ups are responsible for completing all missed exams no later than Friday, January 15, 2021.  Arrangements must be made directly with the teachers from the affected classes.
